Clinical To act as a role model for clinical staff which addresses patient needs and ensure the delivery of safe patient care. Keeps accurate, contemporaneous records and written reports as required. Reports and proactively supports training and development issues arising from clinical and non-clinical incidents. Proactively manages risk to vulnerable persons and supports training and development of staff as identified through development plans. Participates / facilitates meetings in response to the clinical development needs of staff. Works collaboratively with clinical professional colleagues and agencies to ensure effective communication in order that professional practice development needs for staff are identified. Manages risk and maintains service delivery through effective communication and liaison with Service Managers, staff and Professional colleagues. Engages constructively with clinicians and students to support, develop and enhance quality standards of clinical practice in the work environment. Provides clinical leadership, development and advice to Clinical staff, patients and carers based on current evidence-based knowledge on end-of-life care. Partakes in clinical supervision of registered professionals as required by the clinical supervision policy. Maintains up to date clinical skills and knowledge in end-of-life care and maintains awareness of professional issues through the appraisal of research and evidence actively seeking out new sources. Demonstrate personal accountability in their everyday practice and an understanding of their responsibility for staff to which they delegate actions. Contributes to the improvement of patient care and patient safety by effectively disseminating and integrating evidence-based guidance into clinical practice, of both physical health, mental health and learning disabilities. To incorporate patient preferences into treatment decisions, such as planning advanced directives near the end of life, including recording on clinical systems RiO, LCR. Ensure that the Oxleas NHS Foundation Trust Core Values are embedded. Education Acts as a clinical and educational expert in end-of-life care, leading in clinical training, education and programmes for the Long-Term Conditions sub-directorate. Identifies learning needs for Clinical Professional staff and develops education programmes around end-of-life care. Identifies practice areas which require evaluation through team development planning and training needs analysis. Effectively manages competing demands of practice and education related to supporting different levels of learning and development. Evaluates the effectiveness of education and the impact on service delivery and patient care through audit and benchmarking processes. Facilitates learning by actively integrating theory to practice development. Proactively creates a positive learning environment within the clinical services which supports and underpins the principle of lifelong learning, work-based learning and continuing professional development. Provides support for learners, mentors and specialist practice students (community nursing pathway) in the education and development process. Promotes and facilitates understanding of relevant national and local policies in end-of-life care. Responsible for the development of an end-of-life skills data base and the development of career and professional development pathways for this service. Clinical skills Health and Well Being Maintain clinical skills to practice at a safe level and ensure that the use and application of equipment is in accordance with Trust standards. Exercise a high degree of professional autonomy and critical judgement. Make professionally autonomous decisions utilising knowledge and skills. Deliver a high standard of clinical and therapeutic care to patients in accordance with the senior practitioner competencies Service Improvement The post holder will work closely with the Head of Nursing to support professional recommendations in relation to service redesign and new ways of working. Act as a change agent, using skills that will motivate and reassure staff through negotiation and training to facilitate the change process. Ensure effective and efficient use of resources, equipment and supplies. Quality/Competence Practice in accordance with your level of competency Maintain own professional registration in accordance with relevant regulatory body standards. Demonstrate on going professional development. The post holder is required to participate in the Trust appraisal process and work towards meeting identified development needs. Facilitate the collection of data.
