The Role
An exciting opportunity has arisen to join Bolton’s Adult South Neighbourhood Team as a Community Assessment Officer. You will work closely with social work and health colleagues to support adults and carers under the Care Act 2014. The role involves completing assessments, care plans, reviews, Mental Capacity Assessments, and participating in Best Interests meetings.

You’ll work with individuals with complex needs, using resources across health, social care, and voluntary agencies to promote independence and ensure appropriate support. The post also includes administrative duties, managing referrals, and offering advice through a busy duty service

About You
We are looking to recruit an enthusiastic and motivated individual who has a passion for working with adults and carers, understanding their needs, aspirations and supporting them to enhance their quality of life. We adopt a strength-based approach to our practice and seek to provide a person centred service. We are looking for resilient practitioners who can manage competing demands and prioritise work effectively. The role requires applicants who are confident decision makers and who can stay calm under pressure.

While in this role you will need to demonstrate commitment and enthusiasm to working closely with those with care and support needs, their families and carers, other professionals and your managers and peers. You will have the ability to develop and maintain relationships with team members, external organisations, service users and carers. You will have the skills to be actively involved in multidisciplinary team working and decision making.
