Report to the Lead Midwife for Patient Safety, with the oversight of the Head of Governance and Quality Improvement, Heads of Midwifery, Operational Managers and Director of Midwifery, any issues that might affect the provision of care or service within the Service Line being supported. Support the maintenance of the Service Line risk registers in liaison with the Lead Midwife for Patient Safety, Head of Midwifery, Operational Managers, Director of Midwifery and the Patient Safety team. Ensure all incident reports involving or affecting Womens Services are actioned as appropriate in accordance with the PSIRF. Identify and communicate high scoring risk issues and incident trends within the Specialty. Produce monthly reports on incident reporting to support Specialty review of trends and facilitate analysis of all incidents and to communicate and share Trust wide lessons learnt and practice changes as required. Ensure that all incidents and near misses are reported by staff within the area of responsibility in accordance with the Trust policy and support the investigation of PSIIs, ensuring appropriate action plans are developed and monitored, and lessons are learned and disseminated. Be conversant with Systems Engineering Initiative for Patient Safety (SEIPS) and human factors, and lead and advise on this method or the most appropriate method/process of investigation. Support 2nd victims (staff members involved in an incident) after an incident via engagement, de-briefs and sign-posting to other Trust resources such as the Lead Professional Midwifery Advocate, Employee Relations and Advice Service. Foster just culture where learning is the central focus. Support specialty activities related to the improvement of compliance with the CQC and MIS Standards and other assessments relevant to the service, including the provision of information for assessment of compliance. Monitor key performance indicators capable of showing improvements in the management of risk Effectively provide feedback on incident reporting to staff within the Specialty Assist Ward Managers/Matrons/Departmental Heads/Leads to develop and monitor compliance with action plan time scales and provide assurances to the CSL Governance Group. Ensure that evidence/documents to be relied upon are up-to-date and comply with Trusts policy. Raise the profile and importance of quality governance and patient safety within the Service Line Promote a learning culture within the Service Line which acknowledges the benefits of effective risk management and patient safety, in providing psychological safety and a safe environment, thereby reducing risk to staff, patients, careers and visitors. Promote an open, honest and trusting culture within the Service Line so that all staff are actively encouraged to report incidents, acknowledge human error where appropriate, review and reflect upon adverse events, and engage in multi-disciplinary learning. Ensure the Service Line properly embeds and implements action plans following PSIIs. Work with the Specialty management team to plan and organise the governance meetings and ensure that all relevant topics and papers are covered at the meeting. Ensure that the Service Line has appropriate arrangements in place to implement Trust procedures relating to governance, such as incident investigation and approval, complaints and claims investigations, risk assessment and risk registers. Liaise with the relevant staff in the Quality Governance teams when additional support or expertise is required. Coordinate and manage the process in place in the Service Line to ensure complaints investigations are undertaken in accordance with Trust policy and that actions and lessons learnt subsequent to a complaints investigation are identified and implemented. Attend complaints meetings in the Division as necessary. Ensure effective communication and liaison with the complaints team within the Divisions Ensure that the Service Line has a clear programme of work with regard to its Quality Governance agenda to include dashboards and programmes of work. Assist the Specialtys management team in preparation of Specialty reports for Quality Governance panels, groups and committees and any other relevant meetings Contribute information or sections to other Quality Governance reports as appropriate. Ensure that Specialtys work programmes support Trust and national priorities such as: Care Quality Commission (CQC) Registration Requirements, Maternity Incentive Scheme (MIS) Requirements, Any other successor relevant frameworks or external assessments Work with the Specialty and relevant staff in the Quality Governance teams in preparation for external reviews or assessments. This may include evidence collation. Ensure that the Specialty reviews the results from any relevant external or internal reviews or Quality Governance project, working with them to make recommendations for improvements in practice, action plans and supporting the implementation of changes to improve outcomes of patient care and patient/carer experience. Support the Specialty in the development and implementation of their quality governance reporting structures including meetings and in the production of reports required in line with the Trusts Quality Governance Framework, such as reports for the Specialty performance reviews and the Patient Safety Committee. Meet regularly with the Lead Midwife for Quality Assurance, Governance and Compliance, Safety and Quality Programme Manager, Patient Safety manager, Clinical Audit & Effectiveness manager and the Complaints & PALS Manager, and the other Quality Governance Facilitators as a group to ensure group priorities are understood and standards agreed.
