Job Purpose The Practice Nurse will provide and maintain a high standard of nursing care for patients, offering advice, health education, and support to promote better health and wellbeing. Working as part of the multidisciplinary team, the post-holder will manage a range of nursing duties, including chronic disease management, vaccinations, cervical screening, wound care, and health promotion, while contributing to the continuous improvement of clinical services within the practice. Key Responsibilities & Duties1. Clinical Responsibilities Provide chronic disease management for conditions such as asthma, COPD, diabetes, hypertension, and cardiovascular disease. Administer childhood and adult vaccinations, including seasonal flu, HPV, shingles, and COVID-19 where applicable. Perform cervical cytology (smear tests) in line with national screening guidelines. Deliver wound care, dressings, suture removal, and minor injury support. Conduct health checks including blood pressure, ECGs, and lifestyle assessments. Offer contraceptive advice, sexual health guidance, and participate in womens and mens health clinics. Provide health education and support self-management of long-term conditions. Support and deliver public health initiatives, including NHS screening and vaccination campaigns. 2. Professional & Team Responsibilities Work collaboratively with GPs, pharmacists, HCAs, and other clinicians to deliver high-quality, patient-centred care. Maintain accurate and timely patient records using the practice clinical system (EMIS / SystmOne). Participate in practice meetings, audits, and quality improvement projects. Contribute to achieving Quality and Outcomes Framework (QOF) and PCN targets. Maintain up-to-date knowledge and clinical skills through CPD and mandatory training. 3. Patient Care & Communication Provide compassionate, respectful, and culturally sensitive care to all patients. Educate patients about their conditions and involve them in decision-making about their care. Promote healthy lifestyle choices and preventative healthcare. Handle patient queries in a professional, empathetic manner, ensuring high levels of satisfaction. Qualifications & Experience Essential: Registered Nurse (NMC registration). Experience in a primary care or community nursing setting. Knowledge of chronic disease management and preventative healthcare. Ability to work autonomously and as part of a team. Excellent communication and organisational skills. Desirable: Experience in cervical cytology, immunisations, and wound care. Knowledge of EMIS, SystmOne, or similar clinical systems.
