Salary: From £28,141 per year pro-rata to hours worked
Other Benefits: Additional enhancement for unsociable hours, essential care user allowance, paid mileage, paid travel time
Hours: 25 per week (variable shifts between 7am 11pm)
Based: Oldham

Role Overview:

This is an exciting opportunity to join our Community Reablement Service which supports individuals while promoting their independence and helping to restore/maintain their ability to remain within the community.

As a Reablement Support Worker you will need to possess excellent communication and organisational skills, be able to prioritise your workload and work well within a team.

You will share our values and have a passion for providing the highest quality care and support, respecting individual’s dignity, choice and preferences.

You will need to hold a full driving licence for this role and have access to a vehicle.


Key Responsibilities:

  • To assist with the monitoring, implementation and evaluation of care plans and reablement programmes.
  • To complete written records as required with the customer, carer and family following discussions regarding agreed goal planning.
  • To provide flexible, personal and practical support (inclusive of personal care needs) which is respectful of customer’s privacy and dignity and in line with their assessed need.
  • Maintain accurate records, contribute to reviews/assessments and team meetings and liaise with other agencies and professionals such as social workers, nurses and therapists when required.
  • Ensure the appropriate use, maintenance and storage of equipment supplied for the customer’s reablement programme. As part of the monitoring process alert a senior staff member if change of equipment or equipment repair is required.
