Crynodeb
Patient Reception & Support Receive patients and guide them through the streaming process. Collect and escort patients to the UTC as required. Provide a professional, welcoming, and reassuring presence. Act as a chaperone when necessary. Observations & Clinical Support Complete baseline observations within 30 minutes of registration and repeat as required Carry out paediatric observations every 4 hours. Monitor waiting patients for safety and escalate concerns appropriately. Escalate clinical concerns to the GP on site, Sister in Charge, or UTC Lead Nurse. Record and close patient cases accurately in electronic systems Point-of-Care Testing (POCT) Undertake approved point-of-care tests in line with training. Record results promptly in patient records. Liaise with clinical teams if tests indicate the need for admission. Patient Safety & Protocol Adherence Adhere to patient influx protocol when demand exceeds safe levels. Ensure no patient is left without observations for more than one hour. If leaving the department without HCA cover, pause streaming and notify GP/Admin team. Support admin team to ensure reception cover during breaks. Teamwork & Communication Work effectively with colleagues to ensure timely patient care. Share patient responsibility clearly when more than one HCA is on duty. Use Cerner initials to identify accountable HCA for each patient. Maintain visibility in reception and waiting areas to support workflow. Information Governance & Data Protection Use individual logins for SystmOne and Cerner only. Record observations and POCT under your own name. Lock computers when unattended and never share passwords. Adhere to BCA and BTHFT information governance and data protection policies. Restrict mobile phone use to emergencies only. Training & Development Participate in training to support ongoing development. Remain up to date with SOPs and trust policies. Engage in safety training and clinical updates. Contribute to a culture of continuous learning and improvement.
