Summary
West London NHS Trust is seeking an experienced and motivated pharmacist to take on the role of Lead Integrated Care Pharmacist within the Hammersmith & Fulham Community Independence Service (CIS).
This is a key leadership position within a dynamic, multidisciplinary team that delivers integrated, person-centred care to patients in their own homes. The post holder will provide strategic and clinical leadership for the pharmacy component of the service, contributing to high-quality medicines optimisation and patient safety across the borough.
This is an exciting opportunity for a clinically skilled pharmacist with a strong interest in integrated care, medicines management, and collaborative working across health and social care boundaries.
The successful applicant will demonstrate excellent communication skills, both verbal and written, with the ability to convey complex clinical and pharmaceutical information clearly and effectively to a range of stakeholders. They will be a confident and effective leader, bringing extensive clinical knowledge and experience in medicines management. A strong commitment to medicines optimisation and a proactive approach to promoting medicines safety will be essential attributes for this.
1. To provide a high-quality patient focused clinical pharmacy service in an Integrated Care setting at West London NHS Trust.
2. To lead on the delivery of a clinical pharmacy service across the Community Independence Service
3. To carry out domiciliary assessments and medication reviews in identified high risk patients with complex medicines needs
4. To work closely and collaboratively with the ECP pharmacy leadership team and service
5. To ensure the successful implementation and roll out of Trust procedures, quality improvement projects and national legislation.
