Crynodeb
Responsibilities of the Advanced Nurse Practitioner. (not exhaustive). Implement health promotion and wellbeing programmes Manage patients presenting with acute or chronic medical conditions Evaluate individual specialised treatment plans for chronic disease patients Identify and manage patients at risk of developing long- term conditions Provide advanced care to patients as required in accordance with clinical based evidence Request, process and interpret pathology services and other tests as necessary Deliver patient care as necessary, referring patients to secondary/specialist care as required Maintain accurate clinical records in conjunction with legislation Ensure coding is used effectively Maintain chronic disease register Prioritise health issues and intervene appropriately Support the team with clinical emergencies Recognise, assess, refer patients presenting with mental health needs Be an extended and supplementary prescriber, adhering to extant guidance Support patients in the use of their prescribed medicines (within own scope of practice), reviewing annually as required Contribute to practice targets (QOF, etc.), complying with local and regional guidance Liaise with external services/agencies to ensure the patient is supported appropriately Delegate clinical responsibilities appropriately, ensuring safe practice (within the scope of practice) Support the clinical team with all safeguarding matters in accordance with policy Deliver opportunistic health promotion where appropriate
