Crynodeb
The post holder will be working autonomously as part of a hub and spoke model and will be expected to be able to use their own initiative and professional judgement although they will be well supported by the surrounding team and through the clinical and professional networks accessible through the community teams. The service currently operates a 7 day a week service through the hours of 8am to 8pm including weekend and bank holidays. This would be negotiated on a pro rata basis. We are looking for a Nurse that has a passion for making a difference to our patients, someone who wants to enable individuals to live their best lives through the use of our reablement service. We are also looking for an enthusiastic and innovative individual who possesses effective telephone and communication skills, IT skills, and exceptional organisational skills.
