The Early Help Hub forms part of Calderdale’s front door for Early Help Support, it works collaboratively across the borough with a range of services to provide support to children, young people and their families.
The Hub responds to referrals made through to the Multi-Agency Screening Team (MAST) that don’t meet the threshold for Children Social Care involvement. In these cases, it’s been identified that the needs of the child and family can be met through a coordinated Early Help Offer.
The Hub is a multi-agency decision making forum who assess the levels of need for children, young people and families to ensure that the right services and intervention are provided to meet their needs at the right time.
The post holder will offer immediate short-term support for up to 12 weeks, working intensively and persistently with targeted families who face particularly complex and multiple challenges. Using a holistic 'think family' approach they will carry out assessments of needs, provide targeted interventions and bring together a coordinated package of support to meet identified need. They will manage a complex caseload, including those subjects to Early Help assessments, Child In Need, Child Protection and Children with Disabilities, in accordance with procedures and support of Children's Social Care.
The ideal candidate will have experience of working with ‘hard to reach’ children and families through an 'outcomes focussed' approach. They should have strong assessment skills, analytical and reflective skills and as acting as a Lead Professional. The role requires the post holder to operate within a framework of both support and challenge.
