Crynodeb
Deliver a community based, comprehensive, specialist clinical service for people affected by Multiple Sclerosis (MS), with an emphasis on patientcentred care and symptom management.To provide people affected by MS and Health and Social Care Professionals with support, education, advice and information in order to managethe condition effectivelyTo provide a timely response to acute exacerbations of, or continued progression of MS and co-ordinate the appropriate level of care to enablethem to remain at home wherever possibleTo assist the MS specialist nurse in the monitoring & review of patients in Northumberland who may be eligible for disease modifying treatments.To assist in the facilitation and delivery of early supported discharge for people with MS.Ensure accurate documentation and patient records are maintained in line with organisational policy & NMC standards.Participate in regular audit/research evaluating practice and participate in local and national research projects.To support the MS Specialist Nurse in the development and delivery of specialist training & education in MS to people affected by MS, healthprofessionals, allied health professionals, social care staff and students.
