About the Role
This is an exciting opportunity for you to join our Family Hub Team. Trafford Team Together is our team around the school approach so you would be working very closely with education colleagues and wider partners. We are committed to delivering the right support at the right time, and in the most effective way for children and families, through a whole family approach to assessment, planning and delivery of evidence based programmes.
About you
You will bring experience of working with children and families through an early help approach. You will have skills in identifying and assessing the needs of children, young people & families, planning, coordinating and leading support and intervention as a key worker. You will also have experience of working closely in partnership with a range of partners to ensure there is a whole family team around approach’. You will have experience of working with diverse communities and a commitment to ensuring that children, young people and families achieve the best possible life outcomes.
