We have an exciting opportunity for a full time Band 5 physiotherapist to join our Pulmonary Rehabilitation team. This is for a 12 month fixed term contract. The successful candidate will play a key role in delivering high quality patient centred care to individuals across Oxfordshire helping to optimise their respiratory health, improve quality of life and support self management. The role consists of the assessment, development and implementation of tailored Pulmonary Rehabilitation programmes. This role offers a high degree of autonomy and clinical responsibility, supported by strong professional leadership and opportunities for ongoing development. We are looking for individuals with a passion for respiratory care, excellent communication skills, with an ability to contribute to service improvement.

This post is currently going through job evaluation so some of the duties of the role may change.

Main duties of the job

To  assess, plan, deliver and evaluate individual programmes of care in a response to planned and urgent need. In partnership with Pulmonary Rehabilitation Lead and Clinical Lead participate where appropriate in programmes of Pulmonary Rehabilitation including:

    • Delivering aspects of education
    • Identifying suitable patients for the programme
    • Supporting patients through the programme with a focus on their long term management
    • To maintain accurate documentation and records in line with Trust policy and NMC / CSP Codes of Conduct.

To ensure service is developed as a needs led service with consideration being given to demographics, partner service developments.

  • To contribute to qualitative and quantitative audit of service and ensure accurate information is maintained both at service and organisational levels.
  • To work with Clinical Lead to develop team/service objectives.

Detailed job description and main responsibilities

Clinical

  • To assess, plan, deliver and evaluate individual programmes of care in a response to planned and urgent need.                  
  • In partnership with Pulmonary Rehabilitation Lead and Clinical Lead participate where appropriate in programmes of Pulmonary Rehabilitation including:
    • Delivering aspects of education
    • Identifying suitable patients for the programme
    • Supporting patients through the programme with a focus on their long term management
  • Anticipatory Case Management for respiratory patients requiring regular admission to hospital where physiotherapy is their prime requirement.
  • To contribute to palliative care of chronic respiratory patients.
  • To develop partnership working with all relevant agencies/service/individuals to support optimum management of the identified client group.
  • To undertake planned specialised procedures where necessary – in line with service developments.
  • To contribute to the development of specialist protocols and care pathways.
  • To maintain accurate documentation and records in line with Trust policy and NMC / CSP Codes of Conduct.

Management

  • To ensure service is developed as a needs led service with consideration being given to demographics, partner service developments.
  • To participate in the organisations annual appraisal process.
  • To support the development of all staff working in the service
  • To understand/be aware of the financial position of the service.
  • To recognise workload pressures for self and others, seeking appropriate support.
  • To contribute to qualitative and quantitative audit of service and ensure accurate information is maintained both at service and organisational levels.

Education

  • To work with Clinical Lead to develop team/service objectives.
  • To actively foster an environment conducive to the learning needs of staff and students.
  • To provide mentorship for pre, post and specialist students and new staff members from a variety of professional backgrounds and organisations.
Person specification Qualifications Essential criteria
  • HCPC registration or pending before start date
Desirable criteria
  • Willingness to undertake specific service training
Experience Essential criteria
  • Experience working within a Respiratory setting or Pulmonary Rehabilitation
Desirable criteria
  • Experience delivering group health education
  • To actively foster an environment conducive to the learning needs of staff and students.
Knowledge Essential criteria
  • • The ability to demonstrate clinical skills with particular reference to managing respiratory disease.
Desirable criteria
  • • Willingness to undertake further education courses e.g. BTS Pulmonary rehab course
