Provide regular supportive conversations with patients living with a suspected or confirmed cancer diagnosis undertaking a Holistic Needs Assessment (HNA) where indicated.Answer non routine phone calls from clinically vulnerable patients using a risk assessment framework to initiate an appropriate response and escalate to the clinical team as required. .Contact patients / carers after discharge from hospital under the instruction of the AOT to review patients and ensure their needs have been met, or whilst awaiting further investigations under the care of the Cancer of Unknown Primary serviceAssist the AOT to ensure investigations are undertaken and reported promptly; escalate delays in results to the AOT in a timely manner and inform the AOT when results are available.Effectively support the Acute Oncology Team to triage alerts for Acute Cancer admissions using all available IT systems including both internal and external routes of referrals. Provide a full administrative service, clerical & secretarial support for the AOS / AOT including maintaining electronic and paper diaries and scheduling meetings e.g. Acute Oncology Operational Group meetings.Maintain the Minimum Data Set (MDS), which records all of the AOS activity. Ensure that the anonymised version of the MDS is sent to the Greater Manchester Cancer Network quarterly (or as directed by the GMCN) in a timely fashion and before the submission deadline.Support the CUP MDT coordinator where necessary with the preparation for ad hoc CUP MDT meetings including booking the room, ensuring case notes are available and any imaging / pathology reports that may be required.Record minutes and action plans of any AOS meetings and distribute to team members.Ensure ward teams and GPs have CUP MDT outcomes, including patients diagnosis (if known) and decisions made at CUP MDT.Effectively support the AOS to triage alerts for acute cancer admissions by accessing all relevant internal and external referral methods; liaise with referrers, wards and departmentsAssist the AOS in reporting of clinical incidents i.e. breaching of the Neutropenic Sepsis (NS) Door to Needle or the Malignant Spinal Cord Compression (MSCC) MRI within 24 hours targets.Support the AOS in accessing and collating all information from relevant sources e.g. case notes, radiology, pathology, and the Christie Clinical Web Portal (CWP).
