Crynodeb

• The Discharge Navigator will act as the main point of contact between the ward teams and TOCH, working to support their Discharge Facilitator to promote an effective and smooth discharge for all patients on their designated wards. Discharge Navigators will spend the majority of their time on a dedicated ward and will be responsible for gathering patient background information, agreeing patient discharge plans with ward MDTs and working with ward staff to coordinate all aspects of the plan.This may include some weekend work.

An exciting opportunity has arisen for an enthusiastic innovative, motivated and dynamic Discharge Navigator who will be working as part of a multi-professional Team to support the smooth running and co-ordination of discharge services.

The post holder will be responsible for facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach. They will coordinate assessments of patients needed and support the discharge plan. The post holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Integrated Discharge Team to overcome and limit these.

The successful candidate will work closely with all ward teams ensuring appropriate referrals are seen and made to local authorities and preventing unnecessary, avoidable delays for patient discharge.

This role will extend to support and ensure that patients who have been discharged feel supported and have received the support that was requested. You will need to have critical thinking skills and be able to communicate effectively. This role will be full time across 7 days so flexibility will be required.





Croydon Health Services NHS Trust provide hospital and community services from a number of community and specialist clinics throughout Croydon.

These include:
• Croydon University Hospital
• Purley Hospital
• The Sickle Cell and Thalassaemia Centre in Thornton Heath
• Community bases at Purley, Sanderstead, New Addington, Broad Green, Woodside
• A minor injuries unit in New Addington
