Our Mission Whether in the hospice, at home or in partnership with another provider in a different setting, babies, children and young people will experience the same high-quality palliative and end of life care, and beyond, alongside specialist family support. SPECIFIC RESPONSIBILITIES Act as a compassionate advocate for children, young people, and their families, maintaining clear professional boundaries. Collaborate with parents/carers to implement tailored care plans that meet individual needs. Provide sensitive emotional and psychological support to families navigating illness, disability, end-of-life care, and bereavement. Deliver high-quality care in line with nursing plans, policies, and procedures. Administer complex clinical interventions (e.g., tracheostomy care, ventilator support, gastrostomy/jejunostomy feeding, suctioning) following appropriate training. Support palliative and end-of-life care both within the hospice and in home settings. Facilitate enriching experiences and memory-making during short breaks and day stays. Maintain professional competence through ongoing training and development. Keep accurate, timely records and escalate concerns or incidents appropriately. Provide nursing support under the supervision of the nursing team. Ensure a clean, safe, and welcoming environment through daily housekeeping duties. Communicate effectively with the care team, reporting any relevant observations or concerns. Uphold confidentiality and data protection standards at all times. Assist in therapeutic activities, group sessions, and life skills development under guidance.
