Crynodeb
The post holder should be able to work autonomously to provide nursing care to patients in their own home The post holder will be responsible for the planning, implementation, and evaluation of individualised nursing care in a range of settings. The post holder will provide health education to enable patients to self-manage, and achieve optimum independence following deterioration or a period of ill health The post holder will be required to support the senior nursing team in the maintenance of efficient and effective delivery of services. Work closely with multidisciplinary teams including GPs, social workers, therapists, and specialists. Coordinate care to ensure continuity and prevent hospital admissions. Communicate effectively with patients, families, and healthcare professionals.
