Crynodeb
To undertake the triaging of mental health referrals meeting the threshold for urgent/crisis service and to establish an appropriate response. To undertake mental health Crisis assessments, formulation and time limited treatment planning for individuals needing an urgent/crisis response To conduct psychosocial mental health and risk assessments in accordance with service policy To develop close working links with other mental health teams and agencies to enable swift and appropriate transfer between services. To contribute to and deliver time limited, co-productive care plans in consultation with service users, families/carers, community teams, primary care and other appropriate service providers. To provide comprehensive written reports as required. To participate in clinical supervision both on an individual basis and team/peer supervision. Also to engage in delivering supervision to more junior members of the team. Maintain clinical records according to professional and Trust standards. To attend and contribute to clinical meetings within SHSC. To work flexibly and provide cover in the absence of other clinicians within urgent & crisis service. Ensure efficient recording of activity and outcome data. To actively take part in and encourage the placement of students within the service.
