Produce and maintain list of all new cancer patients diagnosed in the Trust by using a variety of sources and IT systems, including: IPM, CRIS, ICE that are held in other departments for example: Radiology and Pathology. Liaising with all Clinical/None Clinical staff within the Directorates. Plan, coordinate and administrate MDT meetings for cancer patients. This will include maintaining a list of patients to be discussed, case note, film and results collection and recording requisite information at MDT meetings, for example: Attendance, records and minute taking which is live recording of data and outcomes. Be able to understand the medial terminology/clinical coding relating to cancer and to undertake training as required. Develop knowledge and experience of relevant cancer tumour sites and understanding of patients pathway. Use knowledge and experience of cancer data to inform/advise colleagues regarding data collection. Be able to collate. Process and develop routine data on a weekly basis to produce timely and accurate information on cancer waiting times and the patient pathways for the Trust, Commissioners, Collaborative and the Department of Health. Continually develop skills in the use of various hospital information systems and ensure training is updated. Minimise risk in the use of sensitive information to ensure that the Data Protection Act is adhered too. Respect differences in relation to equality and diversity ensuring colleagues and patients rights are not infringed. Keep up to date with national and local issues relating to cancer and the collection of cancer data. Implement and contribute to MDT operational policies. Support clinical teams in developing clinically useful, accurate and complete cancer datasets. Contribute to surveys and audits. Staff should be flexible in their approach to working hours. Key Tasks Track patients through their pathway from referral to treatment. Follow up on any diagnostic testing and ensure the appropriate staffing are aware of the patients pathway. Ensure all tracking is kept up to date on a daily basis. Working closely with information Services and clinical areas to monitor cancer waiting times and inform appropriate managers of any changes. Ensure patient is seen with the appropriate timescale in order to meet national standards by liaising with Clinicians, Nurses and Administration staff regarding the patients pathway and highlighting potential breaches at MDT meetings. Ensure all casenotes and relevant paper work is available for MDT meetings. Sending referral information onto other Trust or Services with information that is relevant to the patients pathway. General clerical duties including filing, photocopying and collating of documents. To cover other cancer MDT meetings and tracking for other areas. Any other duties required within the context of the grade.
