Total Social are working with Camden Council who are looking for an experienced locum Adult's Social Worker for their Mental Health Team.
Role involves:
* care coordination of complex cases under CPA
* developing co-produced care plans with service users
* risk assessment and management
* holistic assessment of mental health and social care needs
* conducting Care Act assessments
* working with carers and wider support networks, partnership working with other statutary and voluntary organisations
* liaison with inatient units
* experience of working with adults with psychosis and complex needs
* experience of Care Act assessments and reviews
* good knowledge of relevant legislation, policy & procedure including Safeguarding, Mental Health Act, Care Act and Mental Capacity Act
