The Role
An opportunity has arisen for 2 x Senior Occupational Therapist (OT) within the Independent Living Service. One post will be based within the main area of the Independent Living Service whilst the other will be based in the Discharge to Assess function of The Independent Living Service. Both are on a permanent basis.
The service supports hospital discharge, community follow-up, and specialist input for people with disabilities across Bolton, working closely with Community Therapy and Social Work teams.
In this role, you will support safe hospital discharges and follow-up care in the community, ensuring individuals have the right support to avoid readmission. You’ll carry out Care Act 2014 assessments, develop support plans, and apply relevant legislation such as the Mental Capacity Act 2005. Working closely with therapy and social work teams, you’ll assess needs, provide equipment, and promote independence. You’ll also contribute to service development, improve user and carer experiences through person-centred planning, and support integrated care through collaborative working.
The 5 from 7 post will receive a 7% enhancement as workers are required to work weekends and bank holidays on a rotational basis. This is to support the delivery of the service 365 days of the year
