Triage referrals appropriately, reviewing patients in a timely manner according to their presentation. Liaise with community teams to prevent avoidable urology admissions to hospital/ED attendances. Perform clinical procedures appropriate to the history and physical examination of the patient, including further invasive testing and treatments requiring highly developed skills and precision, including (but not limited to) catheterisation, cannulation, peripheral arterial blood gas sampling. Analyse multiple sources of data including patient history, physical examination and investigation findings when making diagnoses, clinical judgements and evaluating care provided; presenting this information to speciality /senior medical staff for advice when scope of practice is exceeded. Autonomously formulate appropriate management plans for patients, formulate clinical decisions and often complex treatment plans to manage acute illness and acute episodes of chronic illness including referral, admission or discharge. Continuously revaluate findings and clinical response to treatment and establish an appropriate ongoing management plan accordingly. Contribute to clinical governance by leading quality improvement, audit and research projects as part of the multidisciplinary service-wide performance and quality agenda. Conduct patient assessments, including medical history and physical examination to diagnose urological conditions. Order and interpret diagnostic tests such as urine, bloods, imaging and other urological investigations to aid in diagnosis. Collaborate with urologists to develop individualised treatment plans based on patient needs and diagnosis. Prescribe medications and treatments for urological conditions including antibiotics, analgesics and bladder instillations. Perform minor procedures such as catheterisation, bladder scan, LA TP biopsy, cystoscopy, stent removal and Botox treatment under supervision or independently as per scope of practice and Trust policies/BAUN guidelines as appropriate. Educate patient and their families about urological condition, treatment options and self-care strategies. Provide counselling on lifestyle modifications, such as dietary changes and fluid management to improve urological health and prevent recurrence of conditions. Address patient concerns and provide support throughout the treatment process, fostering a collaborative and patient centred process. Coordinate with other healthcare providers to ensure continuity of care for patients undergoing surgical interventions or specialised procedures. Schedule and conduct urology follow-up appointments to monitor patient progress, assess treatment efficacy and address any complications or side effects. Adjust treatment plans as needed based on patient's response and clinical findings Please also see full job description for details of responsibilities required for the post holder.
