The role
Ensure there is a robust referral and initial assessment process, which responds to the needs and timescale of the commissioning authority. Ensure the care package is completed and returned in detail and again within a tight deadline.
Ensure new service users are orientated into their surroundings and introduced to other service users and staff. All paperwork is to be completed on the day of admission including a 72hr care plan from the identified needs of the assessment. Make arrangements for the service user to be seen and registered with a GP the next day and make sure that all medication is prescribed and available. Ensure that relatives, friends etc. are contacted and encouraged to visit the service user., and the appropriate documentation is completed on time.
Following the initial 72hr period, determine if there are any obvious needs to address. Allocate a member of staff to spend time with the service user getting to know them and understand their preferences, to build on the pre-admission information.
Ensure that all service users have care/goal plans which are person centered, Philosophy driven and outcome focused, which can be measured against the four Philosophy outcomes. Devise a system whereby the service user is empowered to be part of their care/goal planning process at all times.
Keep excellent records against each care/goal plan and in general. Ensure effective communication between staff in written and electronic format. Records will be maintained in accordance to the NMC guidelines.
Continuously evaluate care needs with other members of the team who work closely with the service user to ensure that the service users’ care is responsive, safe and effective. Care provided must always meet the needs of the service user and reviewed on a regular monthly basis as a minimum.
