Crynodeb
The post holder will assess the suitability of referrals and ensure eligibility criteria is met. They will be involved with the discharge process to ensure a seamless, timely transfer from hospital to the home environment and communicate with families to prepare them for the transition home. The post holder will risk assess patient needs, environment and care required during each visit. The post holder will need recent Neonatal experience within a hospital setting to ensure they have the clinical knowledge to identify deterioration or recognise improvement of the patient and adapt care plans accordingly. They will need to be confident and competent with nursing skills which including blood sampling, NGT insertion and measuring observations. As an autonomous practitioner they must have the ability to solve problems, confidently function in a lone worker capacity, effectively manage time and demonstrate excellent communication skills. The post holder will practice in a holistic, non-judgmental, caring and compassionate manner that avoids assumptions, supports social inclusion and respects individual diversity. They will work in partnership with the MDT, service users, their carers and families. The post holder will contribute to the practice development within their clinical care setting and work in accordance with Trust Policies, Procedures and Guidelines and will be responsible for completing any work-based training associated with the role.
