Summary
Role Overview As an Experienced Advanced Nurse Practitioner (ANP) / Advanced Clinical Practitioner (ACP), you will: Conduct face-to-face consultations with patients from Sheffield and the surrounding areas. Manage acute presentations to the Walk-in Centre as an autonomous practitioner; support patients with acute, undifferentiated problems as well as chronic long-term conditions; and refer patients to appropriate pathways when necessary. Provide patient-focused, evidence-based care to a consistently high standard; assess, diagnose, and plan care for patients, including prescribing medications (where qualified and clinically indicated). Use your clinical expertise to determine urgency and guide patients presenting to the Walk-in Centre toward the best care pathways. Support the wider multidisciplinary team (MDT) to ensure patients receive care from the most suitable professional. MSc or equivalent in Advanced Practice. Advanced Physical Assessment and Consultation Skills – Level 7. Minor Illness – Level 7. Non-Medical Prescribing Qualification. At least 3 years of experience post-Level 7 qualifications in primary or urgent care. Paediatric Course – Level 7 (desirable). Ability to assess and treat patients with a wide range of conditions. Excellent communication skills across all levels of the organization. Strong organizational skills and the ability to manage a dynamic workload effectively. We have excellent links with the specialists at the Royal Hallamshire Hospital, Northern General Hospital, The Children’s Hospital, Eye Casualty and Sexual Health Centre, who provide support to our clinicians and the patients attending the Walk-in Centre As part of the developments to the service, we have created a wellbeing hub for patients who are struggling with low level mental health and other social issues. We have seen this grow month-on-month since its inception and are starting to expand the service due to its success.
