Job Description Post Title: Practice Nurse Hours: Up to full-time hours available (flexible working considered) Salary: Competitive, dependent on skills, qualifications, and experience Responsible to: Lead Nurse / Partners Location: Queen Square Medical Practice (Lancaster & Caton sites) Qualifications: Registered General Nurse (RGN) Job Purpose To provide high-quality nursing care, health promotion, and chronic disease management within the practice, working as part of the Primary Health Care Team. The post-holder will support patients in managing their health, promote wellbeing, and help prevent illness, in line with practice protocols, current evidence-based guidelines, and the NMC Code of Professional Conduct. Key Responsibilities Clinical Undertake holistic assessment, planning, implementation, and evaluation of nursing care. Provide chronic disease management clinics (e.g. Asthma, COPD, Diabetes, Hypertension, CHD, Epilepsy, Mental Health, Rheumatoid Arthritis). Deliver immunisations and vaccinations (childhood, adult, seasonal, and travel). Perform cervical cytology screening in line with national guidelines. Provide wound care, dressings, suture removal, and management of minor injuries. Conduct health screening and health checks (e.g. new patient checks, NHS health checks, vascular risk assessments). Support patients with long-term conditions, encouraging self-management and lifestyle change. Assist GPs with minor surgery procedures when required. Carry out diagnostic tests and monitoring (e.g. ECGs, peak flow, spirometry, blood glucose, INR). Deliver travel health advice and vaccinations, supporting the practice travel clinic. Provide contraceptive and sexual health advice within scope of practice. Identify mental health concerns and signpost or refer to appropriate services. Professional Responsibilities Maintain accurate and timely patient records, both written and electronic. Ensure confidentiality at all times. Work collaboratively with colleagues across the multidisciplinary team. Maintain professional registration and participate in revalidation. Engage in regular clinical supervision, audits, and reflective practice. Participate in clinical governance, quality improvement, and achieving QOF/IIF targets. Take responsibility for ongoing professional development in line with NMC and practice requirements. Leadership & Teamwork Support, mentor, and supervise Health Care Assistants (HCAs). Contribute to service development and implementation of new protocols. Share responsibility for stock management, vaccine storage, and infection control. Participate actively in practice meetings and training sessions. Administrative & Organisational Assist in maintaining registers for chronic disease management and recall systems. Record all clinical activity accurately on the clinical system (EMIS Web). Work within Health & Safety and infection control policies. Contribute to the smooth day-to-day running of treatment rooms and clinics. Provide cover for nursing colleagues and HCAs during absences where possible.
