About the Role
Lewisham Neighbourhood 1 Community Mental Health Team is seeking a dynamic and compassionate Case Management Officer (CMO) to join our multi-disciplinary team. This temporary role is ideal for someone who thrives in fast-paced environments and is passionate about empowering mental health service-users.
As a CMO, you’ll carry a caseload of clinically stabilised individuals, acting as their primary point of contact. You’ll review strengths-based care packages, support transitions from specialist services to GP-led care, and champion independence through community resources.

What We’re Looking For
✅ Experience
Direct work with mental health service-users and carers
Promoting independence through strengths-based approaches
Knowledge
Support planning and personalised outcomes
Impact of mental illness on individuals and families
Care Act 2014 and adult social care legislation
Strengths-based assessment and care provision
Aptitude
Co-producing care plans with service-users and carers
Risk assessment and management
Collaborative working across teams and disciplines
Effective time management under pressure
Knowing when to seek support and escalate appropriately

Observational and review skills for vulnerable adults
Confident IT user across multiple systems
Strong written communication and numeracy
Influencing and coordination to ensure timely support
Clear, empathetic communication with all stakeholders
