Summary

Location: Based in Tetbury | Covering Gloucestershire and supporting Bristol
Salary: £25,000~£27,000 depending on experience per annum
Hours: 37.5 hours per week | Full-Time | Permanent

We are currently looking for a proactive Care Coordinator~planner to join our Tetbury-based team. This is a dynamic role, playing a vital part in maintaining our standards of care and service efficiency across Gloucestershire, with some involvement and support from our other office based in Bristol.
Role Overview
This office based role would suit someone who thrives in a fast-paced, people-focused environment. Supporting and ensuring compliance, quality, and continuity of care services delivered by our dedicated team of highly trained and experienced staff.
Key Responsibilities
Initially meeting with clients and staff to gain a full understanding of the care visits needed by the client and meeting with staff to understand what hours they require each week
• Ensure all staff are delivering safe, high-quality, person-centred care
• Respond to any urgent issues in the field and support with a resolution
Coordination
• Liaise with clients, families, District Nurses, GP's, hospitals etc to support with any appointments and requirements needed
• Manage and update care rotas using internal systems
• Assist with last-minute shift cover and emergency bookings
• Maintain accurate staff compliance records and documentation
• Contribute to audits, CQC compliance preparation, and internal reviews
• Participate in the shared on-call rota as needed – one evening per week and on weekend per month (After completion of probation and learning the role)
About You
Essential:
• Previous experience in a care coordination or supervisory role (domiciliary or agency setting) minimum of 1 year
• Sound understanding of CQC requirements and safe care delivery
• Excellent people and communication skills, with a calm and professional manner
• IT literate and confident in using rostering or care management software
• Full UK driving licence and access to a car ~ willing to travel
• Time management ~ be able to plan your work efficiently and in a timely manner
Desirable:
• NVQ/QCF Level 3 in Health & Social Care (or working towards)
• Knowledge of the healthcare recruitment sector
• Familiarity with areas across Gloucestershire and Bristol
