Crynodeb
The Community Mental Health Service supports adults living with severe and enduring mental health difficulties in the community. As a Band 6 Community Mental Health Nurse, you will: Deliver psychosocial, evidence-based interventions, including solution-focused work, motivational interviewing and recovery-focused care. Work proactively with individuals experiencing relapse, using a strengths-based approach to reduce dependency on mental health services. Develop and communicate recovery care plans and risk management plans, ensuring information is shared appropriately with relevant partners. Liaise with a wide range of stakeholders, including voluntary sector partners, primary care, housing providers and safeguarding services, to provide holistic and joined-up care. Use your professional expertise to support colleagues through supervision, reflective practice and contributing to team development. Maintain accurate, up-to-date clinical records on Trust systems and ensure adherence to Trust policies, professional standards and governance requirements. Participate in regular supervision and appraisal, and keep up to date with current research, policy and best practice in mental health care. Actively promote the Trust's values of Kindness, Respect and Teamwork in all aspects of your role.
