To provide clinical and psychosocial interventions for young people with complex substance misuse and mental health problemsTo provide guidance and support to members of the team regarding standards of care for patients To provide advice, guidance and leadership in respect of health and concurrent substance misuse needs and mental health awareness. To provide clinical support to team members and care staff at Clayfields when working with young people who have complex substance misuse and mental health needs To attend and participate in team meetings, helping to promote new ideas, methods and assist in their implementation. To care co-ordinate a complex case load including those with significant mental health needs, with responsibility for assessing, planning, implementing and evaluating patient care. To assist in the development of systems, pathways and policies which promote delivery of services to co-existing mental health/ substance misuse diagnosis Children and Young People. To undertake comprehensive assessment of patients needs following referral to the service particularly those who have identifiable mental health issues. To plan programmes of clinical and psycho-social interventions using evidence- based practice. To decide upon and recommend the best course of action and plan care for co-existing Mental Health and Substance misuse patients. To provide information and advice about drugs, alcohol, smoking cessation and related issues, to include blood bourne viruses, safer sex and mental health. Provide ongoing assessment, planning, implementation and evaluation of substance misuse programmes, utilising relevant clinical tools, both clinical and psychosocial To maintain accurate healthcare records in line with NMC Guidelines and Trust policy To co-facilitate group work relevant to substance misuse , offending behavior and mental health. To ensure care plans are formulated , implemented and reviewed. To effectively liaise and develop links with services within the secure childrens home setting and in the community to establish effective through care and after care pathways. To participate in Clayfields House induction programme. To effectively plan discharges and releases in conjunction with community services Promote and maintain harm reduction strategies in the treatment and prevention of associated communicable diseases e.g. Hepatitis and HIV/AIDS Maintain good practice in handling and disposing of contaminated waste and equipment, according to current policy, utilising all opportunities to attend training sessions and updates in the area. Ability to assess and respond to clients at risk of suicide or self-harm using the appropriate clinical intervention and secure setting procedures, drug screening and management of symptoms. To attend and actively participate in team meetings, organised training sessions and mandatory training To be involved in appraisal systems, clinical and managerial supervision, and be active in identifying aims and objectives for personal development to meet team objectives To lead in the training and development of others eg new members of staff, care staff , students and visitors to the service. Maintain own registration requirements in accordance with professional body regulations. To actively liaise with integrated mental health team to ensure consistency in approaches of treatment and case discussions or patients. Attend multi-disciplinary team meetings alongside Clayfields care staff. To evaluate effectiveness of service through audit techniques. Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
