To assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs Chronic Disease Management to include: Diabetes, Asthma, COPD, Chronic Heart Disease, Hypertension, to develop and implement care plans for patients with Long term conditions, promoting self-care. Cytology, Childhood immunisations, NHS Travel vaccinations, phlebotomy, womens health, injections To keep accurate clinical detailed health records on Emis. Undertake mandatory and statutory training requirements.
