Don't miss out on this exciting opportunity to join the expanding urgent community and virtual ward service to support and keep patients safe in their own home.

We are a team of Occupational Therapists Physiotherapists and advanced clinical practitioners working closely with the multidisciplinary team to prevent further deterioration in patients’ physical functioning and wellbeing. We cover the areas of Chorley, Preston and South Ribble.

Your role will be to support the team including undertaking non-complex assessments and carrying out treatment plans under the direction of the therapists.

We are looking for a flexible Technical Instructor/ Assistant Practitioner who is able to work across a range of settings and has experience working to support Occupational Therapy and Physiotherapy interventions within rehabilitation, rapid assessment and/or admission avoidance.

The role involves working with patients in their own homes and a variety of community settings . The service will operate 365 days a year, 8.00am - 8.00pm.

We are looking for a Technical Instructor/ Assistant Practitioner with excellent communication skills and who has the ability to work autonomously as well as part of a team.

You will have regular supervision and professional development opportunities.

This post is subject to an enhanced DBS Disclosure and the successful applicant will be required to pay for their DBS check.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
