We are delighted to invite applicants to join our Urgent and Emergency Care (UEC) directorate and become part of the Emergency Department and Same Day Emergency Care (SDEC) pharmacy team.
This post is patient-facing and involves working alongside our established clinical pharmacy team to deliver a specialised service to our ED and SDEC. You will have a key role in supporting with medicines reconciliation, counselling patients and working with the team to manage the ward automations systems, helping to ensure the effective running of the Pharmacy support with the Emergency Department and SDEC.
You will be fully supported by a team of experienced Pharmacists, Chief Medicines Management Technician, a Medicines Management Technician and Assistants.

In this role you will have leadership and clinical responsibilities and will be one of the Senior Medicines Management Technicians (MMT) leading in patient care and ward automations systems management for the Emergency Department and SDEC.
You will report directly to the Chief Medicines Management Technician for ED and SDEC and assist in implementing service improvements within the department.
You will deliver patient-facing clinical pharmacy care to the ED and SDEC areas and will work closely with the UEC Specialist Clinical Pharmacists to ensure the delivery of a safe, efficient, high quality and cost effective technical service.

Patient medication history-This will include compiling medication histories by checking with patients, their own medication, patient’s medical notes, smart cards, Nervecentre, System One, GP surgeries, recent discharges and any other appropriate sources of information.
Communicating effectively, clearly and concisely complex or sensitive information to a wide range of staff, healthcare professionals and patients where there may be barriers to understanding.
Checking patients own medicines for suitability for use on admission and at discharge.
To complete final accuracy checks of discharge prescriptions against contents of patients own drugs locker.
To be responsible for ordering further supplies of medication as per departmental procedures.
To maintain up to date handovers on NerveCentre via the pharmacy IPads or laptops.
In liaison with pharmacy stores, maintain and review drug supply issues relevant to the clinical area to ensure an effective and safe system of drug supply, through effective communication with relevant pharmacy and ward staff.
To participate in the controlled drug retrieval of patients own medication from wards, patient’s own CD destruction at ward level and expired or unused ward stock. To check CD register balances and balance transfers to new CD registers. To help support the quarterly CD audits.
To investigate and resolve with nursing staff where appropriate controlled drug discrepancies to ensure accuracy and legal requirements of the controlled drug patient records are maintained.
To be responsible for the local implementation of standards relating to medicines storage and security including:
· Ambient and fridge temperature monitoring
· Security of medicines storage in treatment rooms, drug trolleys, fridges, emergency trolleys and boxes, controlled drugs cupboards.
To participate and support in project works under the direction of the Lead Directorate Pharmacist.
The post holder will be expected to take full part in the 7 day shifted rota and bank holidays duties.
Shifted and bank holiday duties: To participate in departmental weekend, bank holiday and late duty
working as required. At the time of advertising, the weekend rota encompasses 2 half-day shifts every 4/5
weeks at the city campus or 1 full day shift every 4 weeks at the QMC campus. In addition, the post holder
will participate in a system of flexible shifts to facilitate NUH pharmacy opening times.
