Crynodeb
You will play a key role in admission prevention, hospital discharge, proactive and preventative care through rehabilitation within the community. You will assess patients on their own homes, in temporary or permanent residential or nursing placements to maximise independence and empower rehabilitation potential. To ensure good communication with clients and carers demonstrating sensitivity in dealing with distressed or aggressive clients To liaise with other team members to ensure the seamless transfer of clients within the pathway In conjunction with other professionals, to work positively towards the development of multidisciplinary, multi-agency approach to clients care, ensuring the highest quality service To participate in multi-disciplinary meetings to enable the appropriate physiotherapeutic elements are integrated in the overall client management plan. To assess client understanding of treatment proposals and gain valid informed consent To respect and ensure the confidential nature of the communication across organisations and with patients or their representatives.
