Our Care Co-ordinator is part of the care team and would be expected to help with care calls where necessary. Key responsibilities of a Care Coordinator: Allocation of care workers to customer visits using effective rostering, ensuring continuity of care and considering locations, skills required.
A Care Coordinator’s responsibilities include consulting with patients and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care.
Ultimately, you will have a compassionate nature, be knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their illness.
Working with the care team to evaluate interventions and identify where and when further ones will be required
Reading, attending workshops and liaising with professional bodies to stay abreast of developments in the field
Develop and coordinate healthcare programs
Communicate between staff, patients and family
Recruit and train staff and create schedules
Develop patient goals and monitor progress
Maintain patient confidentiality and quality care
Developing care plans and rearranging them as necessary when difficulties arise
Visiting patients, checking on the care they've received and documenting it accordingly
Job Types: Temporary contract, Part-time, Full-time, Fixed term contract
Contract length: 12 months
Part-time hours: 40 per week
