This is an exciting opportunity to join the Specialist Palliative Care and enhance Palliative care skills experience and expertise within the Specialist Palliative Care field
Delivering high quality palliative care to patients within the Community setting and with the potential to rotate into the Acute Hospital setting across North Cambridgeshire.
You will have knowledge of symptom control for patients with end of life malignant or non-malignant care needs and be able to assess the holistic palliative care needs of a Patient and their family
The job will involve the education and training of other health professionals regarding end of life and specialist palliative care across North West Anglia NHS Trust and the local community.
In supporting a seven day specialist palliative care service the Trust region and the local community, the successful candidates will need to work weekends and Bank Holidays pro rata.
The role offers an opportunity to develop specialist palliative care knowledge and experience across the hospital and community provision in the region.
This role is community based initially, and candidates will be required to travel between to Community based patients and will therefore need to hold a current car or motorcycle licence and have access to a vehicle.
To deliver high quality holistic palliative care interventions to patients with complex palliative care needs, their families and support care teams.
To demonstrate nurse leadership in action, role modelling Trust values whilst delivering patient focused care.
To deliver teaching formally and informally to patients, carers and colleagues.
To engage with service improvement tools, including audit to support service innovation and audit.
Developing Skill and expertise and delivering plans of care delegated by the Specialist MDT.

Use existing knowledge and expertise and enhanced levels of clinical judgement to undertake complex and holistic assessments within the scope and level of practice expected of a Palliative Nurse.
Act within the scope of practice and expertise to plan, deliver, monitor and evaluate care using clinical assessments, diagnostics, and interventions.
Act as a resource within their own organisation and for external agencies.
Develop, deliver and evaluate education and training opportunities for others within own scope of practice.
Communicate effectively in challenging environments and situations with patients, their families/carers and the multi-disciplinary team.
Encourage and engage with innovative clinical practice to support a culture of excellence within the wider health and care team.
Manage unpredictable and unplanned clinical situations with support from senior MDT colleagues.
Participate in resource management, service development, planning and service improvement.
Develop and appraise staff and learners in line with expected level of competence.
