Crynodeb
Accepts continuing responsibility for the assessment of care needs and maintenance of high standards of care by: Organise team to provide individualised patient care by utilising skill mix appropriately within the available resources. Ensure all staff work within national and locally agreed policies and procedures. Instructing and communicating changes to other health care professionals, of patient condition/ needs as they occur. Provide and deliver health education and health promotional advice within the Pre-assessment Clinic and general ward areas. Engage, interact and teach carers and families to be actively involved in patients care. To ensure patients, carers and family have the relevant knowledge and skills to continue with care after discharge. For further details please see the attached JD & PS
