Crynodeb
1. To ensure all CKD patients starting renal replacement therapy have had all the appropriate educationto allow them to have an informed choice of their options for renal replacement therapy2. Mange the CKD team in the absence of the manager3. Undertake the co-ordination and the management of CKD patients with renal anaemia4. Implement individual education programmes for patients and carers entering the pathway5. Assist with continuing development of health promotion for CKD patients6. Undertake the continuing development of the End of Life care (EOL) for CKD patients7. Constantly updating one own knowledge , recognising the importance of a critical approach to researchand developments in nursing8. Be actively involved in research & clinical audit within the clinical area of CKD/renal9. Assist in identifying all potential recipients for transplantation work these patients up for referral10. Participate in the transplant on call rota11. Work collaboratively within the multidisciplinary team and participate and participate in the induction ofnew staff to renal
