An exciting opportunity has arisen for a permanent, full-time Continuing Healthcare (CHC) Coordinator within theLeadProfessional Team. If you hold a qualification in Social Work, Occupational Therapy, or Nursingand have a proven track record of working effectively with partners to apply the NHS Continuing Healthcare Frameworkwe would love to hear from you. Applications from candidates with a nursing background are particularly welcome. About the Role In this dynamic role, you will work alongside the current CHC Coordinator, bringing your operational experience and expertise in NHS Continuing Healthcare to further strengthen the team.This is an ideal opportunity to balance direct practice with leadership and development responsibilities. As CHC Coordinator, you will: Supportstaff in applying the NHS CHC Framework. Jointly manage complex cases with adult care practitioners. Ensure guidance, training, resources, and practice standards are in place. Monitor CHC activity using appropriate systems. Represent Lincolnshire Adult Care on panels,supportdispute resolution, and maintain effective relationships with NHS partners.
