Main Duties & Responsibilities 1. To accurately and promptly carry out the instructions of the Community Paediatrician or Registered Nurse (RN) who is professionally accountable for the delivery of care. 2. To book appointments for and perform the pre-clinic appointments which includes but not limited to anthropometric measurements, general concerns, and holistic assessments. 3. To book appointments for patients to attend follow-up appointments with the Community Paediatric service. 4. To greet, interact with, and emotionally support the patients and attendees of the Childrens Centre 5. To provide reception cover at the Childrens Centre, responding to patient and attendee queries, and answering the telephone 6. To report to registered staff any patient that gives cause for concern or any observed changes in a patients clinical condition. 7. To support patients emotional well-being by supporting positive conversations. 8. To accurately document all care delivered to patients, including recording documentation electronically on trust systems maintaining confidentiality at all times. 9. To respond appropriately to clinical emergencies to ensure the patient receives appropriate care. Be aware of role in emergency situations and what actions should be undertaken. 10. To be aware of and adhere to all trust policies / procedures / nursing guidelines when undertaking any activities. 11. To participate in the admission process under the supervision of registered staff. 12. Provide and deliver a high standard of individualised personal nursing care, including measuring height, weight, and anthropometry; recording fluid balance and nutrition; measuring heart rate and blood pressure; wound observation; and urine analysis. 13. To assist and take responsibility for maintaining medical devices and clinical equipment, including decontamination, taking devices to families and keeping accurate records. 14. Be responsible for the ordering of medical / surgical supplies and ensure these are stored correctly. 15. Maintaining a safe working environment. 16. To adhere to trust infection control procedures, including the cleaning of toys and clinic rooms. 17. To assist in the preparation of patients and equipment for treatment, investigations or procedures as directed by a Community Paediatrician or RN. 18. To chaperone/escort patients to other clinical departments and or NHS Units under the direction of the Community Paediatrician or RN. 19. To support relatives under the guidance of a Community Paediatrician or RN. Communication 1. To welcome patients, visitors, and relatives to clinical areas where required, providing assistance, support and information as needed. Being aware of barriers to effective communication and respond appropriately. 2. To answer the telephone, when necessary, in a polite courteous manner and respond appropriately within remit and pass on enquiries to the Community Paediatrician or Registered Nurse. 3. Communicate effectively within the nursing team, multi-disciplinary team and other departments to ensure that patient care is well planned and coordinated. 4. To contribute to the handover of the care of patients within the team under the direct supervision of a Community Paediatrician or RN, maintaining confidentiality at all times. 5. Report and record using the trusts systems (Datix) of all incidents, accidents, and complaints to the senior nurse on duty. 6. Use the System One and ICE and other trust systems as required by the clinical area Management 1. Promote a professional, welcoming and uncluttered environment, keeping all storage areas well stocked, safe, clean and tidy, in accordance with any relevant legislation. 2. To be responsible for the maintenance of all medical devices used within the clinical team, including maintaining the service log, liaising with patients, and families where monitors require servicing, delivering and collecting equipment form patients, with oversight from a registered nurse. 3. Work collaboratively with the integrate care system when patient equipment / medical devices require purchasing, delivering and servicing. 4. Ensure all waste is disposed of in line with trust policy. 5. Participate in audit under the direction of the Community Paediatrician or RN. Professional/Educational 1. To ensure up to date with all mandatory (including if appropriate safeguarding and safeguarding supervision) training sessions. 2. To have completed the Care Certificate or relevant experience in a care setting 3. To be flexible in relation to patient requirements. 4. To continue personal development in relation to identified objectives. 5. To supervise and support new staff and working with local induction programme 6. Maintain up to date knowledge of changes in policies and procedures relevant to the post 7. Participate in training and supervision sessions relevant to the clinical area this could include clinical competences where appropria 8.
