Job Summary: You will support and assist the Board of Directors in the provision of patient care. You will be responsible for the provision of skilled nursing care to the population of York Central locality. You will deliver care within the boundaries of your role, focusing on supporting patients to be healthy, monitoring long term conditions, health prevention and screening activities. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care, supporting the delivery of policy and procedures. You must always work within your own competence and professional Code of the Nursing and Midwifery Council (NMC). Duties and Responsibilities: The duties and responsibilities to be undertaken may include any or all the items in the following list. Duties may be varied from time to time under the direction of the Medical Director or Board of Directors, dependent on current and evolving workload: Clinical responsibilities: Assess plan, develop, implement and evaluate programmes of care to improve health and well-being and meet the healthcare needs of individuals. Perform and interpret spirometry (ARTP Registered) Perform routine nursing tasks: Clinical observations Height/ Weight Provide a holistic approach and deliver evidence-based care. Administer and monitor medication consistent with protocols, standards and legislation. Enable patients, families and carers to develop their knowledge and understanding about their health and well-being and be as self-managing as possible. Prioritise health problems and intervene appropriately to assist patients in complex, urgent or emergency situations, including initiation of effective emergency care. Assist in the provision of nursing care within the context of primary care for patients with mental health problems. Offer advice about adult and child vaccinations. Delegate aspects of care clearly and appropriately, adopting the principles of safe practice and assessment of competence. Ensure that all daily checks are recorded accurately and in a timely manner e.g. Spiro/FeNo equipment, defibrillator, emergency equipment, oxygen. Other responsibilities: Recognise and work within own competence and in accordance with the professional Code of the NMC. Support should be provided to other clinical team members as required Monitor the safety and effectiveness of own clinical practice. Maintain accurate and complete documentation and records utilising IT as appropriate. Use own judgement, resourcefulness and common sense. Follow agreed protocols, referring to GPs as appropriate. Follow local and national Infection Control guidelines. Assist with the maintenance of equipment and report defective equipment Take responsibility that any equipment either clinical or otherwise, used when working, is taken due care of and locked away at the end of every working session. Maintain stock levels of clinical equipment and supplies, reporting the Reception Team as required. Assist in setting up the Community Diagnostic Centre at the start of the session and ensure equipment is appropriately stored at the end of the shift. Business responsibilities: Act as a positive role model. Maintain ethos and culture of Nimbuscare. Positively promote Nimbuscare. Support Line Managers as required. Adhere to Health & Safety and educate staff within your role. Apply Nimbuscare policies, standards and guidance. Attend in-house governance, educational and staff meetings as appropriate. Suggest and support the introduction of new working processes to optimise quality. Communicate to ensure that all employees operate as an effective team. Build and maintain relationships with other healthcare professionals, including GPs, GP surgery personnel, care home personnel, CCG personnel, Students. Recognise the types and signs of abuse and neglect. Ensure the Nimbuscare Safeguarding Lead is alerted to, and kept fully informed of, any concerns you may have in relation to safeguarding adults, children and young people. Adhere to Nimbuscare and local Child Protection and Safeguarding Adult policies and procedures. You will participate in any training programme implemented by Nimbuscare as part of this employment, such training to include: Participation in individual performance reviews, including maintaining a record of own personal and/or professional development. Working in conjunction with senior management, assess own learning needs and undertake learning as appropriate. Make effective use of learning opportunities within and outside the workplace, evaluating their effectiveness and feeding back relevant information. Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments. Undertake mandatory and statutory training as required. Understand own role and scope in the organisation and identify how this may develop over time. Work as an effective and responsible team member, supporting others including students and exploring the mechanisms to develop new ways of working. Accept delegation from senior staff, prioritise own workload and ensure effective time management strategies are embedded in own practice. Participate in team activities that create opportunities to improve customer care. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. Participate and support local projects as agreed with the management team. Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.
