Crynodeb
The key functions of this role are to provide medicines reconciliation; promote safe & effective use of medicines in community settings for people accessing the Virtual Frailty Ward (VFW); to be a resource for other members of the MDT; to function as an effective member of the VFW MDT & actively participate in reviewing the effectiveness of the role & how this might evolve to deliver the most positive outcomes for this group of patient
