Crynodeb
Work closely with women and their families to plan and coordinate their care based on their individual needs. You will provide a link between the maternity service and other services involved in the womans care planning. You will provide a link between the maternity service and other services involved in the womans care planning. You will work in partnership with a multi-agency and multi-disciplinary team from health, perinatal mental health and social care to provide high standards of care for women and their families. Youwill act at all times in a manner that upholds the Trusts values and Nursing & Midwifery Professional Framework goals, working as part of a team to ensure that women and relatives receive excellent care with compassion.
