Main Job Purpose: To provide a specialist palliative care physiotherapy (PT) service to patients and carers accessing Royal Trinity Hospice services Key duties and responsibilities Provide a patient-centred specialist physiotherapy service to palliative care patients admitted to the inpatient unit (IPU), attending out-patient services or in the community To undertake comprehensive (holistic & multi-system) assessments and formulate a treatment programme through shared goals with patients and carers, maintaining functional, realistic and measurable (SMART) goals, including risk assessment To work within the Therapies team, consisting of physiotherapists, occupational therapists and complimentary therapists To work closely with other team members and supervise junior members of staff, including the palliative care assistants, therapy assistant, students and volunteers To work independently and as a part of the inter-disciplinary team (IDT), applying clinical skills in physiotherapy, treatment planning, goal setting, intervention and evaluation (use of outcome measures) and includes individual and group orientated approaches Contribute to the evaluation and continued development of the Therapies Service in line with Therapies departmental and Royal Trinity wide objectives/ work plan Communication and Relationships To work as an active member of the IDT, understanding profession specific roles, liaising and attending IDT meetings and relevant clinical and case planning meetings Communicate effectively at every level within the organisation and in the external environment, maintaining effective working relationships with colleagues and peers Manage individuals and groups (staff, patients, formal meetings and the public) including those who are emotionally distressed, angry, and/or in conflict with the information given to them To receive and convey sometimes highly complex and highly sensitive information, including detailed clinical or corporate information To keep contemporaneous patient records, skilled in written report preparation and maintain accurate, clear & concise clinical records (SOAP notes). Provide verbal and written reports to the clinical team and other relevant parties, adhering to local procedures and professional standards: Hospice, Chartered Society of Physiotherapy (CSP) and Health & Care Professions Council (HCPC) related to documentation (paper and/or electronic notes) Model the values and culture of the organisation in interactions and relationships at work Use excellent interpersonal skills to quickly establish effective supportive and therapeutic relationships with patients and their carers Communicate clearly with the inter-disciplinary team during meetings and in caseload reviews to expertly manage the care of patients referred to the service and facilitate handover /discharge where appropriate To ensure that all the hospice clinical and administrative policies and procedures are followed To demonstrate excellent communication skills with patients, their carers and members of the IDT. All interactions should be underpinned with empathy and a clear understanding of the effects of the patients condition Describe and discuss all patient services, both clinical (e.g. IPU, CNS) and supportive (e.g. Wellbeing, Compassionate Neighbours) to patients and stakeholders Please find more details in the Job Description and Person Specification attached
