Have responsibility for the assessment, planning, delivery, and evaluation of evidence-based quality care to an allocated group of patients on a shift basis. Work in partnership with patients and those that matter to them, involving them in decisions about their care. Recognise any deterioration or change in a patients condition which may require the intervention of others and refer as appropriate. Work collaboratively as part of the multidisciplinary team, establishing and maintaining effective communication and relationships. Provide early intervention to ensure the safe discharge of patients, working in collaboration with other relevant teams and services to prevent unnecessary readmission of patients. Act as an advocate for the patient and their family, protecting their interests and statutory rights, whilst promoting privacy, equality, diversity, and inclusion. Maintain a professional image in accordance with the uniform policy and uphold the Trust values and behaviours, promoting One Culture of Care. Direct and supervise the work of non-registered colleagues including, but not limited to, healthcare support workers and student nurses. Undertake additional training to ensure competence to undertake any extended skills or specialty-specific clinical procedures as required. Adhere to the NMC Code and Calderdale & Huddersfield NHS Foundation Trust policies. Use resources effectively and efficiently. Monitor standards of care and escalate any concerns in line with established processes to the Ward Manager or Matron. Where concerns are unresolved, the use of Freedom to Speak Up and Whistleblowing processes should be instigated. Act as a Link Nurse or Champion within a defined speciality/ interest area as required. Be involved in Trust initiatives to improve patient safety. Engage and participate in meetings at ward, divisional or Trust level as necessary. Be flexible to work within areas or departments outside of base area to meet service demands or changing service needs. Provide mentorship, preceptorship and support the education of newly registered nurses, new staff members, and students, facilitating an effective learning environment. Ensure essential safety training and role specific training is completed in line with Trust policy. Take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with revalidation requirements. Contribute to the safety and quality agenda by reporting any identified risk and ensuring actual incidents or near misses are reported. Receive feedback from patients and those that matter to them, recognising and sharing good practice, as well as managing any verbal complaints, escalating to senior team when necessary. Participate in the investigation of incidents/complaints as required. Anticipate, recognise, prevent and manage issues of conflict, escalating to ward manager if unresolved. Assist in the professional development review and appraisal process for more junior staff. Participate in audit and research programmes, analysing findings and implementing appropriate changes to enhance the care experience for patients and those that matter to them.
