Crynodeb
To plan, implement and evaluate nursing interventions within the community, including educational settings. To support the transition processes for children with complex health needs, taking the lead where appropriate. To involve parents and children, encouraging them to make informed choices concerning their health and well-being. To work without direct supervision and be responsible for the management of own workload. To participate in the identification, referral and support of children aged 0-19 years with complex health needs. To maintain and to further develop own professional practice in line with the needs of the service. The post holder will have a special interest in Tissue Viability and specialist training and the development of this across the targeted nursing team.
