Crynodeb
Job summary The post holder is responsible for the delivery of general practice nursing services to the registered practice population. The focus of the role is on the provision of evidence-based care in respect of disease prevention and early detection, disease monitoring and management, health promotion, supporting acute health needs and management plans, and providing proactive support and shared management for patients with long-term conditions. As an autonomous and competent practitioner the post holder is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management and monitoring of patients health needs. In addition, they will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nursing leadership where required.
