The Greenhouse is an inclusion practice managed by East London Foundation Trust. We register adult patients who are experiencing homelessness(this encompasses street homeless, hostel dwellers, those in temporary accommodation and those who have no secure housing). Patients registered are offered the full range of primary care services. We also provide an outreach service in Hackney and the City, providing clinics in hotels, hostels and shelters looking after vulnerably housed service users and rough sleepers. The role is based in both the practice (I day) and the outreach hotel settings(2days)
1. The post holderis responsible forensuring the delivery of safe and effective nursing care to the practice population.

1. The post holder is accountable for nursing servicedelivery,and they will lead and manage the Health Care Assistant.

1. Clinically, the focus of the role is the delivery of evidence-based care for patients with long-term conditions, wound care and management and preventative nursing interventions to all patients.

1. As an autonomous practitioner the nurseis responsible forthe care delivered,demonstratingcritical thinking and skills in clinical decision-making

1. They willdemonstratesafe, clinical decision-making and expert care for patients within the general practice.

1. The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing leadership and direction for the nursing team.

1. The post holder will build effective working relationships with other health care professionals.

1. The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice.

• Promote health and well-beingthrough the use ofhealth promotion, health education,screeningand therapeutic communication skills

• Manage and treat long-term conditions, in line with national and local policies and practice/ network guidelines

• Review medication for therapeutic effectiveness,in accordance withevidence-basedpractice and national/practice protocols, and work with patients to support adherence to prescribed treatments

• Prioritise health problems, intervening appropriately, toassistthe patient in complex, urgent or emergency situations, including initiation of effective emergency care

• Support and manage health needs of women presenting for family planning and cervical cytology consultations

• To provide a sexual health screening service to patients whereappropriate includingsignposting and onward referral as needed.

• To Manage to Blood Borne Virus screening and vaccinating programme within the practiceand outreach settings

• Implement andparticipatein vaccination and immunisation programmes for both adults and children

• To deliver up to date travel health advice for adults and children

• Meet the needs of patients presenting for opportunistic and ongoing wound care, working in partnership with the local Tissue Viability Service asappropriate.

• Diagnose and treat minor illnesses

• As an independent prescriber: prescribe safe,effectiveandappropriate medicationas defined by current legislative framework and local policies

• To clearly communicate the health status of the patient usingappropriateterminology,technologyand format.

• Provide safe, evidence-based, cost-effective, individualised patient care.

• To work within the practice guidelines,policiesand protocols.

• Tomaintainaccurateand contemporaneous records, utilising computer systems whereappropriateand consider the Caldecott Principles in relation to all data handling.

• To be a resource of specialist advice, specifically relating to the common emotional and behavioural problems, to colleagues


within the service, other professionals and agencies/organisations working in the field.

• To support the development of interventions, consultation and psychoeducation approachesregardingprevention and resilience building and health promotion.

• To activelyparticipatein the delivery of QOF targets and local enhanced service targets
