An exciting opportunity has come up for the Adult Community Nursing Service We are looking to appoint a Community Matron to work 3 days per week, who is passionate about high quality patient centred care to join our highly regarded Community Matron Team. The service operates from 7am until 7pm across the Twickenham borough.The post is advertised as a 6 month fixed term contract to cover a secondment within the team and is for 22.5 hours per week.
The post holder will need to provide line management and leadership for a locality nursing team, being accountable for the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.

· To be responsible for the delivery of the locality nursing service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working.
· To provide line management and leadership for a locality nursing team, being accountable for the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.
·To work in partnership with the primary health care team, social servicesand the wider multi-disciplinary locality team, providing holistic assessment of health needs and the provision of nursing care to housebound people with a range of medical needs and long term conditions.
