The Role
We are seeking a dedicated and experienced registered Social Worker to join our Hospital Discharge Assessment Team (HDAT) in the role of Senior Social Worker. HDAT provides adult social care support, in line with the Care Act 2014 and with regard to the Mental Capacity Act 2005, to residents of North Somerset who have ongoing social care needs and are being discharged from hospital after an in-patient stay. We follow the Hospital Discharge and Community Support guidance.
The HDAT management currently comprises of a Team Manager and four Senior Social Workers. You will be joining an established and experienced Senior team who will support you to settle into the post. Your responsibilities will include:
Key Responsibilities:
Supervision of a number of Social Workers and Adult Social Care Workers
Along with the other Senior Social Workers, supporting the team to enable a smooth and efficient person-centred journey from hospital to home for residents of North Somerset who have care and support needs
Lead Social Work practice learning and development and quality assurance within the team
Support staff undertaking ASYE and student placements
Formulate collaborative working with Health and other internal and external partners
This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.
Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.
Our office location is the Town Hall, Weston super Mare.
Work Style

However, please note that during the induction period, it may be necessary to spend more time in the office to support your learning and development. This role falls into the category of Mobile/Field Worker which means you can expect to work in the following way:
0-1 day in the office
Includes field workers who primarily work remotely.
Comes into the office for meetings or focused work. Virtual connections with the team are essential, with portable technology supporting work on the go.
About You
We are looking for a qualified, SWE registered Social Worker who is passionate about their work and who possesses the skills, knowledge and abilities to undertake this role. You should be able to work well in a team, in a busy work environment, and be able to adapt to unpredictable and challenging levels of demand of the service. You should be able to deputise for the Team Manager when required and work together with the other Senior Social Workers to oversee the day to day operation of the team. You should be able to manage your workload efficiently and provide responsive support and guidance to team members managing complex cases often with significant levels of risk. You should be able to work effectively with people and their families in difficult circumstances, and be able to guide and support them through unfamiliar processes. You will need a positive and solution-focused attitude to create positive outcomes within the limits of resources available.
